DESCRIPTION:  \n  \n  \n  \n  \n  \n  \n  \nShared decision-making is central to person- and family-centred care\, especially when supporting older adults with dementia and multimorbidity at home. This session explores how respecting and valuing the diverse perspectives of individuals\, families\, and care providers can improve collaboration and lead to decisions that align with what matters most to the person. Participants will learn practical\, evidence-informed strategies to integrate cultural values\, communication preferences\, and lived experiences into care planning.
