DESCRIPTION:Be a part of the Palliative Care Heart Disease Community of Practice!  \nThis series provides a valuable opportunity for interprofessional palliative care clinicians and heart specialists with a shared passion for palliative care to connect. Participants will engage with a panel of experts in the field to discuss real-life cases and explore key topics\, including symptom management and various models of care\, such as home care\, hospice\, and palliative care units. \nThis 1-credit-per-hour Group Learning program has been certified by the College of Family Physicians of Canada for up to 6.0 Mainpro+ credits. \nRegister for the next session of this series\, “Diuretic management in an outpatient setting” happening on December 11\, 2024\, from 12–1:00 p.m. ET. \nREGISTER HERE
