DESCRIPTION:On October 2\, 2024\, the Heart Disease Community of Practice returns for its third series! This series provides a valuable opportunity for interprofessional palliative care clinicians and heart specialists with a shared passion for palliative care to connect. Participants will engage with a panel of experts in the field to discuss real-life cases and explore key topics\, including symptom management and various models of care such as home care\, hospice\, and palliative care units. \nThis 1-credit-per-hour Group Learning program has been certified by the College of Family Physicians of Canada for up to 6.0 Mainpro+ credits. \nDon’t forget to register for the first session of this series\, “Collaboration Building: How to build collaboration with teams in your setting” happening on October 2\, 2024\, from 12–1 p.m. ET. \nREGISTER HERE
