CHCA Project ECHO Integrated Seniors Care
Shared decision-making is central to person- and family-centred care, especially when supporting older adults with dementia and multimorbidity at home. This session explores how respecting and valuing the diverse perspectives of individuals, families, and care providers can improve collaboration and lead to decisions that align with what matters most to the person. Participants will learn practical, evidence-informed strategies to integrate cultural values, communication preferences, and lived experiences into care planning. A live panel discussion will illustrate how interdisciplinary teams in primary care, home care, and community services work together to navigate complex decisions with empathy, respect and person-centred approaches.
