Heart Disease Community of Practice – Series 3
On October 2, 2024, the Heart Disease Community of Practice returns for its third series! This series provides a valuable opportunity for interprofessional palliative care clinicians and heart specialists with a shared passion for palliative care to connect. Participants will engage with a panel of experts in the field to discuss real-life cases and explore key topics, including symptom management and various models of care such as home care, hospice, and palliative care units.
This 1-credit-per-hour Group Learning program has been certified by the College of Family Physicians of Canada for up to 6.0 Mainpro+ credits.
